Requirements for LSHSC Screening Exercise 2025
The following documents are to be presented by candidates for the screening exercise;
- Printed Application Form/Slip: The confirmation slip generated during the initial online application process, often containing an ID number.
- Invitation Letter: An official letter from the LSHSC, which specifies the screening venue, date, and time.
- Academic Certificates: Original and photocopies of all relevant educational qualifications (e.g., SSCE, WAEC, NECO, ND, HND, B.Sc., M.Sc., etc.).
- Birth Certificate/Declaration of Age: A valid document, typically endorsed by the National Population Commission or a Local Government Council.
- Certificate of Origin: A valid certificate of state/local government origin, duly signed by the Local Government Chairman or Secretary.
- Valid Means of Identification: A government-recognized ID such as a National ID card, National Identification Number (NIN) slip, driver’s license, or international passport.
- Medical Fitness Report: Evidence of physical and mental fitness from a government-recognized medical hospital.
- Letter of Good Character: Evidence of good character, often from a village or district head of the applicant.
- NYSC Certificate: For graduate positions, a National Youth Service Corps (NYSC) discharge or valid exemption certificate is mandatory.
- Passport Photographs: A specified number of recent passport-sized photographs.
